Swansea Bay Children’s Community Nursing Team are currently looking for Band 4 Health Care Support Workers skilled in the care of children with highly complex health care needs, to join our Childrens Community Nursing Team. In order to achieve our vision for the future we currently have posts available for Band 4 HCSWs who possess essential levels of practical skills and knowledge in caring for children with complex needs, and have knowledge in leadership and teaching skills.
Applicants must have QCF level 4 qualification and experience in caring for children with complex needs at AFC band 3 or equivalent.
Main duties of the job
Please note this position is predominately night work and requires lone working in the family home.
The Assistant Practitioner /Senior Children’s Community Nursing Support Worker will work under the direction of the Registered Nurse to deliver high quality, holistic, compassionate health care to children and young people with complex needs, providing the required support for families. The post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a registered practitioner with appropriate supervision in place. Having an understanding of evidence based practice and delivering care in line with current evidence, the post holder will take responsibility for taking action relative to an individual’s health and care in accordance with organisational policy and procedures. The post holder will be responsible for some elements of assessment, implementing programmes of care and modifying individualised care plans, reporting back to the registered practitioner. They may delegate work to others and will supervise, teach and assess other staff.
