Job summary

An exciting opportunity has arisen for Band 3 HCA to join the Transplant Outpatients team.

This experience will allow the successful applicant to expand their existing skills and knowledge in this area of clinical practice.

The successful applicant will work alongside the transplant Nursing Team team supporting service delivery Monday to Friday in an outpatient settings.

Opportunities will be given for the development of advanced knowledge in caring for patients with Heart and Lung transplant, mechanical circulatory devices and pre-assessment patients. This will give the successful applicant insight into the role of a Transplant Nurse in order to support the team and contribute to the patient pathway.

Main duties of the job

Main tasks & responsibilities:

  • To carry out care and treatment, delegated from professionally qualified staff, requiring a wide range of technical ability, numeracy and literacy skills as well as an excellent level of interpersonal skills.
  • To supervise, where appropriate, less experienced Health Care Assistants in the clinical area ensuring patient focused care is provided.
  • To provide any administrative and clerical support to the nursing team.
  • To be flexible in the approach to the clinical area and provide cover for any areas specified (i.e. reception cover during leave periods)
  • To ensure that the clinic environment is prepared each day for the patient journey to run smoothly.
  • Under the guidance of a trained nurse to ensure notes are prepared for clinic and the appropriate investigations are ordered.
  • To be aware of the national guidelines on booking appointments and giving patients choice.
  • While covering Reception, to book follow-up appointments for patients attending the Transplant Outpatient Department in line with Trust Policy and protocols for clinics, whilst considering patient choice.

You will work with a team of Transplant Nurses who are responsible for the management of heart and Lung Transplant patients.

The successful candidate will need to be flexible in their working practice and possess good communication and interpersonal skills. Our Health Care Assistants undertake a variety of interesting roles, which include phlebotomy and reception duties. Training will be provided.

Job responsibilities

The post holder will work under the direction of the Transplant nurse in Mechanical Circulatory Support and will include;

Supporting the Nurse Specialist in the management of patients with Mechanical Circulatory Support (MCS). This will include assisting the Nurse Specialist with the management of the patient in an inpatient and outpatient setting.

Learning how to assist the Nurse Specialist in complex situations with patients with MCS.

Liaison with nursing staff and MDT to deliver individualised, compassionate care to patients, care givers and companions.

Work with the Nurse Specialists to facilitate high quality, individualised care to patients/clients and relatives in collaboration with the multi-disciplinary team.

With supervision from a Nurse Specialist, assist with clinical procedures and investigations.

Assist the Nurse Specialist in the preparation of tests and result for outpatient clinic.

Safely and competently escort stable and designated MCS patients to and from departments within the hospital and local vicinity.

Escort stable and designated MCS patients to other hospitals for procedures.

To competently complete and document basic driveline dressings and report any changes to a Nurse Specialist as necessary.

To learn to competently set up theatre trolleys for MCS implants.

Ensure that equipment is cleaned, re-stocked or disposed of as per policy.

Ensure phone and e-mail enquiries to the MCS office are dealt with appropriately and in a timely fashion.

To assist the Nurse Specialist with the collection of information from local hospitals including blood results, procedure results and clinic letters.

To assist with safe and well communicated patient discharge into the community.

To assist in the preparation of paperwork for national VAD database and other research projects. To participate in collecting the necessary data for these projects.

The post holder is expected to carry out all relevant forms of care with support of the specialist nursing team and clinical specialists.

Education and qualifications

  • oGood use of spoken and written English

Desirable

  • oGSCE or equivalent oLevel III NVQ in Health oCare certificate

  • Balance competing priorities and meet targets , oGood understanding of infection control guidelines, oExcellent organisational skills,

Desirable

  • oSkills in Microsoft Office, Word, Access, EPIC, Excel and PowerPoint oPhlebotomy training
