Job Summary

We have various exciting opportunities in the Stockton on Tees 0 – 19 Service working within our Health Visiting Teams for Healthy Child Practitioners / Community Nursery Nurses/Early Years Practitioners.

Main duties of the job

The Healthy Child Practitioner is a member of the 0-19 Integrated Public Health Service (IPHS), working within the policies and procedures relating to the 0 – 19 service.

The post holder will work closely with the SCPHN within a skill mixed team to support the delivery of the 0-19 Service Specification and Healthy Child Programme (HCP).

The aim of the service is to provide universal access to children, young people and families to the Healthy Child Programme and facilitate early identification of additional and or complex needs.

The post holder will contribute to the Public Health Agenda, to support the promotion of health and reduction in health inequalities. They will work in in partnership with key partners and agencies to meet local and national agendas

Qualifications

Essential

  • NVQ Level 3 Qualification or equivalent.
  • Experience of working with and supporting parents and families.

Desirable

  • Experience of facilitating group work.
  • Working with a range of statutory and voluntary organisations
