As a leading UK Academic Health Science Centre, we at Imperial College Healthcare NHS Trust pride ourselves on being at the cutting edge of innovation and service development and delivering world class clinical outcomes. We are seeking enthusiastic and motivated professionals to join the Open Access Follow Up Service; a team of 5 Cancer Support Workers, line managed and supported by our Macmillan Patient Support Supervisor. This stratified follow up service supports patients to self-manage, reducing hospital attendances and providing better patient experience, while still safely providing regular monitoring for cancer recurrence. Our team are based at the Charing Cross site, working 8am – 4pm, Monday to Friday.
This is a great opportunity to take on a diverse role incorporating direct patient contact and support delivered over the telephone, as well as administrative duties including data management, case coordination, booking appointments and patient tracking.The ideal candidate will have experience of working with a multidisciplinary team, a good understanding of cancer pathways, advanced communication skills and excellent administrative skills. Previous experience of using Somerset and Cerner systems would be desirable.
